Request a Copy of Your Medical Record
You have a right to access your medical records, and to have your records (or a portion of your records) released to another party. The forms below begin the process.
This is known as the "General Authorization for Use or Disclosure of Protected Health Information." Please print this form, complete it and deliver it in person to Medical Records Department located on the first floor of the hospital. Please be sure to bring a valid photo ID.

Eden Medical Center will provide access for inspection during business hours within 5 working days of receiving the request. Staff will provide copies within 10 working days of receiving the request and any applicable fees. Birth Records
If you are seeking a copy of your birth certificate and you were born at Eden Medical Center, or anywhere within Alameda County, birth records are maintained by the Alameda County Clerk-Recorder's Office at 1106 Madison Street, Oakland, CA 94607, 1-888-280-7708.
You can also order vital records online from the Alameda County Clerk-Recorder's Office.
Eden Medical Center does not issue vital records such as birth certificates. The County Clerk is the only place where you can obtain a copy of your official birth certificate.
